Simple Consent Form Template

Standard forms are never too old! Sometimes you just need a very basic consent form with no extra design or extravaganza. And here it is: a classic white background, formal font and a short set of fields. We have included only the questions that are most frequently used in general consent: name, contacts, address, signature, a statement of voluntary agreement and Terms and Conditions checkboxes.

The template can be used in tons of activities and areas, considering you can easily customize it to your own purposes. Make this blank consent form your base and extend it with more questions and fields. Collect online responses, store and save them as PDFs and print out results whenever you need — it’s easy with AidaForm Expert!

It’s easier to start with the Use This Template . If you are an AidaForm user, this general consent form will be carried over to your account in a second. No account yet? The Use This Template button will take you to the registration form. Don’t skip this step, especially as it’s free. Right after, the form will be moved to your account where you will discover the amazing AidaForm editor, tools, and services.
2
Get comfortable in the Builder and explore the two main toolkits on the sides: the Form Elements and Form Designer tabs. The first one contains a whole range of form fields for your convenience. Want to add an element? Drag-and-drop it to the template and type in your data. Or, you can remove some of the elements. All the changes will be saved automatically.
3
Even though we conceived the template as a generic sample, you can still customize the design. The Form Designer contains all the necessary tools to spice up the form with some color and design twists. Color schemes, numerous fonts, button styles and icons are at your disposal.
4
To start actually using the form, go to the Publish tab, switch on the form, and it’s ready to be shared. Since it’s an online form, you send out a link or embed it on your website so people can access it with mobile devices and laptops/desktops.
5
OK, you have your basic informed consent form. But what about the responses? All of the submitted forms will be carefully stored in your Response Inbox unit. There, you can also download or print out responses. All the collected data, including eSignatures, will be on paper. Make sure your form header and texts are transferred to the PDF responses — to do that, check the Set Up -> PDF tab.
