Get comfortable in the Builder and explore the two main toolkits on the sides: the Form Elements and Form Designer tabs. The first one contains a whole range of form fields for your convenience. Want to add an element? Drag-and-drop it to the template and type in your data. Or, you can remove some of the elements. All the changes will be saved automatically.

Even though we conceived the template as a generic sample, you can still customize the design. The Form Designer contains all the necessary tools to spice up the form with some color and design twists. Color schemes, numerous fonts, button styles and icons are at your disposal.

To start actually using the form, go to the Publish tab, switch on the form, and it’s ready to be shared. Since it’s an online form, you send out a link or embed it on your website so people can access it with mobile devices and laptops/desktops.

OK, you have your basic informed consent form. But what about the responses? All of the submitted forms will be carefully stored in your Response Inbox unit. There, you can also download or print out responses. All the collected data, including eSignatures, will be on paper. Make sure your form header and texts are transferred to the PDF responses — to do that, check the Set Up -> PDF tab.
