Start with the Template

Add the ready-made template to your AidaForm account — it’s the fastest way to create a blog survey without building one from scratch.

Customize Survey Questions

Open the template in the Form Builder and adjust it to your needs. Edit titles and descriptions, then add, remove, or reorder survey questions. Free AidaForm accounts offer multiple question types, such as multiple choice, dropdown, rating, and slider scales, so you can easily expand your blog feedback form. Simply copy the template questions and add your own to better understand your readers’ preferences. Use page breaks to organize questions into short, clear sections and create a survey that’s friendly and easy to complete.

If you have an Expert account, you can also add Logic jumps to show different pages or custom thank you messages based on respondents’ answers.

Adjust Design and Settings

Open the Design tab to update the color scheme, button shape and font to align with your branding. Then go to the Set Up tab to enable email alerts and receive notifications about new responses.

Publish and Share your Form

When your survey is ready, click Publish to generate a shareable URL for readers, newsletters or social media. To embed the form directly into your blog, open Publish, select Embed on a web page, copy the code and paste it into your blog editor. AidaForm works with major platforms like WordPress, Wix, Squarespace, Blogger and Webflow. On WordPress, paste the code into a custom HTML block; on Wix, use the embed widget. The form will automatically adapt to your layout and mobile screens.
