Log in to the AidaForm Service

Create a Survey Form

In your member’s area, click Create a Form and choose a template you want to work with. Select the Blank template if you want to only the fields you need for your survey. Alternatively, you can go for the Basic template that already has some standard fields like Name or Email. No matter which template you choose, you’ll be able to add or delete all blocks in the Form Editor. Select a template, type in the name of your form, and click Create.

Customize Your Survey Form

Open the Form Editor, and go to Form Elements. Here you can add the fields you need for your online form: Full Name, Email, Phone Number as well as your survey questions.
